Do you have any scars,
tattoos or other distinguishing marks and/or characteristics?
Some time ago I filled out a form that asked me that question, and I’m damned if I can remember what it was, but it sticks in my head that it was an old form 4473. Anyone remember if a 73 ever asked that question?

No, not on any 4473 that I remember. The last time I remember being asked for scars, etc., was when I was in the Air Force and filling out forms for a Top Secret clearance.
They ask for that information on the forms for concealed carry in Indiana. Not enough blank space for descriptions IMO.

I think I might be tempted to wiseass in the mode of Mohammed Ali: “Distinguishing marks? Well… I am exceedingly pretty.” Can’t you just hear him saying that?

I just started at the top and worked my way down. But I’m sayin’ there’s not enough room if you have more than two or three ‘distinguishing marks’.
Having just recently crushed my left index finger between the panels of my garage door, whilst closing same, I can now update my “distinguishing features”.
I have now added a nicely splintered fingertip, a soon to be missing nail, one hell of a ruptured-tissue scar with a matching set of eight stitches.
In other words, it got smashed like a hammer hitting a grape.
Learned not to let my hand slip off of the handle when closing the damn thing! Just FYI, I took the stitches out about an hour ago. (incident was 12 days ago.) Six of them came out smoothly. Two of ’em kissed nerve on the way out, reminding me just how badly the damn thing got hurt!
Locally, when folks ask what I did to my finger, I tell ’em it was a catastrophic nose picking mishap.

Once, I was compelled to give those answers. I have no tattoos, but I made damned sure he listed every scar I have. I’m damned proud of each of them. Ended up using the blank side of the form. Silly form only gave two lines for them. I pointed out the ones I had sutured myself, too.
All my scars are on my midriff, which is almost always covered. I listed them anyway. Might as well be thorough.
Yeah, Jim, did the Doc pull a couple of the knotted ends out the wrong way also?
Few years back daughter looked at my hands & forearms and said “If you ever disappeared and they asked about distinguishing scars or marks, they’d get an awful long list.”
A lot of the burn scars have faded, but…
If I loose my right leg I loose my distinguishing marks… LOL
I believe that that when trying to get an Airman’s Certificate, one is asked if he has any distinguishing scars, tattoos, etc.
